In my regular partnership we go out of our way to bid our shape on hands like this.

I think pard has a 3-5-1-4 with game forcing values and bid 4 now.

Variations like 3-5-0-5 or 3-6-0-4 happen too but the above is most frequent. Since pard has a game force, they don't have 4 and 5 or they would have bid 2 first.

I'm not surprised if others play differently but we have had a thorough discussion on these.

I like ggwhiz's treatment, but playing online with a new partner, it's not what I'd expect.

I think partner is making a fourth suit bid, and has only four hearts. He/she holds something like Txx - AJxx - Ax - KQxx and is looking for help to play in no-trumps.

I'd bid 3NT.

Without special agreement, I'd just assume slammish and spade control. Anyway I'm bidding 4 cause if partner can make something more opposite this, I'm sure he'll bid again.

I give partner a strong hand, slammish.

I believe he was using 3! as an artificial "NMF" bid, ostensibly asking for 3 cards .

The 3! was his cheapest Ctrl Cue.

Show your cheapest Ctrl by bidding 4 next.

The great thing about this holding is that I don't really have to know whether partner is probing for notrump or slamming in hearts or diamonds. 3NT patterns my hand, doesn't show strength, and is prepared for a pass or more bidding.

I expect partner to pass, though; and I will see the dummy shortly. If he doesn't, the final contract will probably be in some number of hearts, and I will find out later what he has --- confident that I have done my part in the auction showing 3-card support and the location of all my high cards.

For me 3S is either a NT probe or initiating slam procedings for hearts. We will find out which after bidding 3NT.
