We play Acol, so 1♣ showed at least 4. 2NT was 19 points balanced, game forcing. 4NT was RKCB for ♠.
ATB Missed slam.

If west doesn't think he can bid the slam missing only one KC, he shouldn't bid 4NT.

Hard not to blame West for not getting to slam.

Nothing wrong with "Leaping 4NT" at times. Better than taking things slowly and bidding 3NT!
The problem is simply West's failure to realise that there were 33 hcp. Slam seems automatic and there is no reason to stop. If opener had ♥ king then 12 is practically a laydown, and if not it is a 50/50.

Edit: We play our 1NT rebid as 15-18, which helps keep bidding lower for invitational hands, so we don't need 2NT to have a wider range.

Hence the difference between conventions.
"Leaping 4NT" is a good convention.
"Idiot Leaping 4NT" is not.

With doubts about the trump suit she should just bid 6NT with 33 hcp. Since she has doubts about she should simply bid 4♦ prior to 4NT. You then will love your hand even more since the K♦ is upgraded and you have decent ♠ and clearly cooperate with her slam move.
Blame 100% West

I see the others 100% and raise all in.
West as declarer even protects against a heart lead.

Partner was right, 6♠ could be very poor (add K♥ and remove J♠ and make the hand a bit flatter with 3 spades for example), but she didn't have the vision to realise 6N would be good. 100% W.
