Have you seen the video of where Arsenal tried the Cruyff penalty in a match they were winning easily and completely messed it up ?
One I've not seen in a shootout (but have seen in normal play) is where the ball hits the underside of the crossbar, bounces in front of the line, but the backspin then takes it into the net, not sure if that's a goal.

If I read things right, the penalty should have counted, but was eventually disallowed in the Italian appeal's process. (It was a game ASV Tramin vs US Dro Calcio if you want to google yourself.)
