This was played with screens and South explained 3H as a shortage to you. East led the 4♥, and declarer played the queen. How do you defend?

play small odds of p having underlead Axx of hearts border on nonexistent after
this bidding and there is a very real threat of p being squeezed in the red suits
if we play the heart K and transfer the main heart guard to our partner.

this depends on partner so much, I know many players who would never underlead an ace here and others who would do it often.

Play the king. If it loses, so what? I doubt we were beating it. But if partner has:
♠Jxx ♥Axx ♦Jxx ♣AJxx he will not be impressed by our wooden defence. From partners point of view, things look pretty bad, but not hopeless. South seems to have good diamonds and spades seem to be running - but there is a chance he can get us in with a heart for a club through. He is not worried about South having the ♥KQ, since there was no 3NT bid.
In before this gets transferred the the L and A section.

As you guessed, or knew, this was a ruling question. North actually had a fragment in hearts, not a shortage, and the former was the system agreement. West played the king, fatally, but the TD and AC adjusted the score. I originally thought that playing the king was just an error, but your proposed layout and cogent argument have convinced me otherwise. So much so that I now think the TD and AC were right and it is not worth putting in the rulings section.

Has anyone ever seen a declarer play the Q from dummy from Q108x when holding singleton J in hand?
