What do you think responder has, and what do you think declarer should do with KTxx AQx Ax AKQx ?

Responder has 5(6) hearts and forgot (or missed a (couple of) heart(s)) to transfer.

I don't know why one would assume a mistake, when a non-mistake is plausible.

If partner has 5♥/3♠ or 6♥/3♠, he could easily opt Muppet Stayman. If Opener bids 3NT showing hearts, the hand is weird, but no problems develop. If Opener shows five spades, the 5-3 spade fit is found. If Opener bids 3♦, 3♠ shows 4(+) hearts and then Responder blasts hearts to show that the hearts were longer. If Opener had bid 3♥ to deny a 4-card or longer major, Responder would presumably blast hearts again.

The question, though, is whether Responder is 3-5 or 3-6. Fortunately, with three hearts you have no problems as far as strain -- hearts it is.

A secondary question is the possibility of bidding 6NT at MP or 7H at IMPs. Opposite Kxxxx in hearts and the spade Ace, you can count 11 tricks. The spade Queen would make both 6NT and 7♥ decent, but there is no way to find this out now. I sense that 6NT or 7♥ might make, but I just do not feel good bidding either.

What I think partner may have usually gives me a headache, but if I have to guess then

Responder has 5+ hearts and 3 spades. Either he forgot, or you did not discuss how to set hearts as suit for this auction, so he blasted the contract.
I would pass 6H, as I dont want to play 6NT with Ax in ♦ opposite possible singelton, with good support for ♥. Also I dont want to raise to 7 when we may very well be missing an ace.

I can't imagine partner having anything else than long ♥. No idea why he's bidding this way (have a drink afterwards to discuss this), but he made his point clear imo. Pass!

I don't know what he has, but when stuff is fishy it's better not to try and make guesses. I'm going to bid 6NT.

Isn't 6NT a guess? See Yu's concern about AX opposite a stiff diamond. I would rather guess to play 6H...when stuff is fishy, AND partner has jumped to a suit slam, I remember something about letting it go.

Yes, something has gone wrong. But my guess is that partner also knows she has done something wrong earlier, and is bailing out to prevent further complications.

Maybe mechanical error on the first round, 3C instead of 3D with QJ KJXXXXX KXXX (void). Or, Maybe she rethought the dubious pleasure of playing the 4-4 spade fit with 4-7-2-0.

I pass and trust PD who has at the worst is offering me a choice between 6♥ and 6NT and I note I've allready denied 4♥ or may have simply missbid earlier for mechanical or other reasons.

Why is responder not 6-4 and offering a choice of strains? That also seems plausible.

You seem to have a great hand to play in hearts for all plausible meanings so its clear to pass.
