at what point has a card been played

If an op has seen a card before it hits the table has the card been played?

Not necessarily.

L45C. Compulsory Play of Card
1. A defender’s card held so that it is possible for his partner to see its face
must be played to the current trick (if the defender has already made a
legal play to the current trick, see Law 45E).
2. Declarer must play a card from his hand if it is
-a- held face up, touching or nearly touching the table; or
-b- maintained in such a position as to indicate that it has been played.


So, for declarer it doesn't matter whether or not a defender has seen the card, if it has not yet been played. For a defender, it is possible for an opponent to have seen the card before it had reached a position where his partner could have seen it.

An op might see a card while it is still in your hand, and that certainly doesn't count as playing it.

Also worth looking at the rules on exposed cards, penalty cards, etc, for a complete view of the laws in this area.
